Once you have made a consultation with Ms. Flammia, please download the following form(s) onto you computer that correspond to your needs. All clients should fill out the Initial Client Interview Form.
Kindly print and fill out the forms with as detailed information as possible. Amounts to the exact dollar are not necessary. The more complete the forms, the more you can make of your consultation time with Ms. Flammia.
Once complete, you can fax, mail or bring the completed forms with you to our office for your consultation.
